Many quests have bonus prizes and secret side missions you can undertake. Information about various secrets that players have discovered over the years is available below.
Please be cautious when sharing this information with new players. Some people like to discover things on their own, and once you know all the secrets a game has to offer, you can never enjoy the experience of discovering something new again. You may wish to think twice before reading everything below...
General Information
These topics apply to multiple quests.
Super Hard Mode

Super Hard mode contains a kill-tracking side quest. The enemies you kill are counted towards a goal of 1 million kills, with a reward of 3 Photon Spheres for every 25000 kills you get. The details are:
Kills are tracked per Guild Card, so all of your characters share the same kills. You can therefore use any character you want to contribute towards your total kills.
Kills are counted in every Super Hard quest, so you can play any quest and area you want.
Kills are counted at the end of quests, so you must finish them in order to get any credit.
Unlike the Maximum Attack quests, your kill total is personal, rather than a shared total of how many enemies your whole team has killed in the quest.
You can check how many kills you and your teammates have at the statistics machine in Garon's Shop.
When you surpass a multiple of 25000 kills, talk to the soldier next to the Lab teleporter in Garon's Shop, and then...

Prospective Horizons
You will receive 1 coin on Normal, Hard, and Very Hard modes. You will receive multiple coins in Ultimate mode, depending on the quest, the number of players in the team, and whether you played Super Hard mode. You can also get Lucky Coins in Garon's Shop by winning multiple prizes in a row at the slot machine.

Lucky Roulette
I've made many changes to Lucky Roulette, including:
You can no longer win Monomate, Sol Atomizer, or Star Atomizer, as these prizes are too weak to justify their inclusion in roulette.
The board now has different figures: there are four boys instead of five and one nurse instead of two. Two girls have been added, instead.
The odds of landing on the nurse have been reduced, as there is only one nurse now, rather than two.
The odds of landing on Paganini or Nol are slightly higher.
The odds of getting good percentages on a weapon from Nol are higher.
Nol's weapon tiers have been adjusted. Rangers can now win Mechguns, while Forces can win Handguns. This gives Rangers the chance to win a useful weapon from roulette.
When you win a weapon from Nol, it will now come as a present using a random wrapping paper, including the next one you need to unlock. You then have the option of taking the present to Zidd to unlock that wrapping paper before you open it, assuming it's the wrapping paper he wants.

Weapons will have 5-40% on Hit, and all weapons are available to all character classes. When you win a weapon, it will come as a present using a random wrapping paper, including the next one you need to unlock. You then have the option of taking the present to Zidd to unlock that wrapping paper before you open it, assuming it's the wrapping paper he wants.
You will also receive a Lucky Coin whenever you win multiple times in a row. If you win seven times in a row, you'll receive the Lucky 7 prize of 75 Lucky Coins.
If you don't win one of the prizes listed above, you'll receive a consolation prize of a Monomate, Meseta, or Garon points, instead.

The side quest for converting monster parts into equippable items has been restored. This feature was originally available in V2, but Sega removed it in V3, thus greatly lowering the incentive to play this quest. As in V2, your goal is to locate the microchip on Ragol and give it to Dr. Montague in exchange for a weapon conversion. Unlike in offline mode, weapons converted in this quest will receive random percentages. The specifics of monster part conversions are:
The team creator must have completed the offline quest Doc's Secret Plan, or Dr. Montague won't help you.
After you acquire the microchip, you must complete the quest in order to perform a monster part conversion.
The location of the red and green players' microchips has been reversed, so that player 1 and player 2 will have their microchips in the same area on both V2 and V3.
Each player in the team can convert one monster part.
Weapons converted in Normal and Hard modes will have 5-30% on one damage area.
Weapons converted in Very Hard mode will have 5-30% on two damage areas.
Weapons converted in Ultimate mode will have 5-60% on two damage areas, as well as a small chance to have 5-30% on Hit.
On average, weapons converted in Normal, Hard, and Very Hard will have 5-10%, and weapons converted in Ultimate will have 15-30% per damage area.
Other changes to the quest include:
The translation of Dr. Montague's dialogue has been improved to make slightly more sense.
If you complete the quest alone, the type of material you get is more strongly influenced by the difficulty. Only desirable material types are awarded in Very Hard and Ultimate modes.
If all four players collect the microchip, the lab worker will reward you with a material based on the current difficulty. (Of course, you would only give her the microchip if you don't have any monster parts to convert.)
If fewer than four players collect the microchip, the odds of which type of grinder the lab worker will give you have been adjusted.

Quest Completion Prizes

Super Hard Bosses
To activate the secret Super Hard boss mode, have all players enter the Principal's Office, and then talk to the left desk. If you defeat all the bosses and complete the quest, you'll receive 6 Photon Crystals.

You will receive Weapons Bronze Badges for playing these quests in Ultimate mode. The number of badges you earn scales based on the number of players in the team. You can trade your badges with the Weapons member on Pioneer 2. The possible trades are:
3 Bronze Badges for 1 Silver Badge
3 Silver Badges for 1 Gold Badge
3 Gold Badges for an item
You will receive a different prize each time you reach 3 Gold Badges. The prizes are:

Each shield's item description displays the Guild Card number of the person who earned it. If you've already gotten all three shields, you'll receive 5 Photon Drops, instead.

Prizes are awarded when you reach 10000, 25000, and 50000 points. The prizes are the same as in offline Challenge mode, as it's difficult to find people to play splitscreen with, and most releases of PSOv3 don't have offline Episode II Challenge mode at all. The prizes are:
The percentages your weapon receives are randomly generated. The possible percentages are:
Normal: no percentages
Hard: 5-30% on one damage area
Very Hard: 5-30% on two damage areas
Ultimate: 5-60% on two damage areas, with a small chance for 5-30% on Hit
Your score resets back to 0 points after you receive a prize, so you can start building your points again.

S Rank
You must meet several conditions in order to complete the quest with S Rank. The conditions are:
You must be in Very Hard or Ultimate mode.
You must complete the quest in less than 90 minutes.
You must complete the quest with either no deaths or maximum kills.
If you want to reach the maximum of 415 kills, you must defeat multiple hidden bonus enemies and activate hidden switches. The required steps are:
Spaceship
Clear the room after the first warp, then head north for an extra Grass Assassin.
Clear the next room, then head north again for an extra Delsaber.
In the room with many walls, enter the dead end in the northwestern corner to spawn an extra Hildebear in the next room.
Palace
In the southwest corner of the map, head north for two extra Sinows in the long hallway that connects to the first room.
Return to the previous room, then go west to find a small room with a warp. Turn around and shoot a button above the door before taking the warp.
Clear the long S-shaped room that's divided down the middle, then head north to a small room with boxes. Turn around and shoot another button above the door.
Return to the bridge room for an extra Chaos Bringer.
After the final room, head west and take the warp on the grassy platform to access the scaffold room. Kill the extra monsters there, then warp back down to a balcony room with a blue laser fence switch. Press the switch to exit the balcony, then head north for an extra Hildebear.
Super Hard Mode
This was the first quest I made that contains a special difficulty setting. As such, the process for activating Super Hard mode is different in this quest. The team creator must have completed the offline quest Journalistic Pursuit in order for you to activate Super Hard mode. Proceed to the Principal's Office and talk to the FOmarl there. She will give you a Technician Access Code that you can use with the terminal in Mine 1. Select the Updates option at the terminal and input the code to enable Super Hard mode.

Quest Completion Prizes
You will receive a weapon from Melvin upon beating the quest. Weapons will have a random special attack and Hit percentage, depending upon the difficulty level:
Normal: Green weapon tier only, up to 20% Hit
Hard: Green and Blue weapon tiers, up to 30% Hit
Very Hard: Green, Blue, and Pink weapon tiers, up to 40% Hit
Ultimate: Green, Blue, Pink, and Red weapon tiers, up to 50% Hit
This gives you the opportunity to get weapons with useful special attacks that don't drop from monsters. The weapon will also be wrapped as a present using the next wrapping paper you need to unlock, so you can show the present to Zidd in Garon's Shop before you open it if you want to unlock more wrapping paper types. If you've already unlocked every wrapping paper type, you'll receive a random wrapping paper, instead.
Christmas Cards
You can get a Christmas Card in Ultimate mode. Complete the quest, then proceed through a fake wall in the Northwest corner of the final room in Mine 2 and talk to the computer there. You will receive a Christmas Card if your team hasn't died too many times. The maximum number of allowed deaths is:
1 player: 4 deaths
2 players: 3 deaths
3 players: 2 deaths
4 players: 1 death
If you bring 3 Christmas Cards to Phil (the boy standing next to the Principal's Office), he will give you a Christmas Present.

Hungry Rappy
After you disarm the probe, you can find a Rappy waiting for you in a nearby room. He will send you on a mission to find mushrooms, which are growing somewhere in Forest 2. If you find some mushrooms for him, he will give you a feather that you can exchange with the fashion-obsessed lady on Pioneer 2 to get an Easter Egg weapon. In Normal, Hard, and Very Hard modes, you will receive a random weapon. In Ultimate mode, you get to pick the weapon you want. The possible prizes are:
Normal, Hard, Very Hard
Male: Sonic Knuckle, Huge Battle Fan, Drill Launcher, Rocket Punch
Female: Panther's Claw, Marina's Bag, Rabbit Wand, Amore Rose
Ultimate

The percentages your weapon receives are randomly generated. The possible percentages are:

Weapons Side Quest: Sil Dragon
You can undertake an additional side quest in Ultimate mode if the team creator is a member of Weapons, i.e. if the team creator has obtained Akiko's Frying Pan from the offline quests. You can speak to the Weapons member by the bank to check your eligibility. If you're eligible, have all players shrink before you disarm the probe, then approach the large red door near the final computer console in Forest 2. If everybody inserts their Key Cards, you'll be able to fight the Sil Dragon. If you succeed, you'll be rewarded with a Weapons Iron Badge. Show the badge to the Weapons member on Pioneer 2, and you'll receive Wok of Akiko's Shop with randomly generated percentages.
Halloween Horror

This quest contains doors that won't unlock simply from killing enemies. You must find and attack hidden switches in each area to open them. The switch positions are:
Ruins 1: Stand near the warp behind the waterfall and look west to find the button high on the wall.
Ruins 2: Stand on the south platform in the final room and look west to find the button behind the electrified column.
Attacking these buttons will unlock an extra door in their respective areas. Go through the bonus doors to press pink door switches that unlock a 2-player door in Ruins 3. Go through this Ruins 3 door and turn around to find the final button just above the door.
In Ultimate mode, activating this button will open the door to Dark Falz. If you win the battle, you'll receive Unknown Residue that you can trade with the scientist on Pioneer 2 for a Jack-O'-Lantern.
The quest also has randomised switches that will force you to fight either in darkness or in poisonous fog if you activate the wrong one. Something in the quest will change to tell you which switch is the correct one to press...
Acrid Aquifer

This quest contains optional 2-player doors in each area. If you defeat the extra monsters, you'll find laser fence switches that open the path to Dal Ra Lie in Ultimate mode. Defeating Dal Ral Lie will increase the number of Photon Crystals you receive when you complete the quest.

This quest awards Photon Tickets, which you can use to purchase various items. You can trade your tickets with Hopkins, who is standing near the bank. The ticket rewards are:
Normal, Hard, Very Hard: 1 Photon Ticket
Ultimate: 2 Photon Tickets
Super Hard: 3 Photon Tickets
The available items are:
Chu Chu Fever: 9 Photon Tickets

You can view the number of tickets you have in the Quest Board, and you can carry a maximum of 10 tickets at a time.

Quest Completion Prizes

Red Ring
To acquire the Red Ring, you must be playing in Ultimate mode. Follow these steps:
Complete the prerequisites in The Ends of Desire.
Reach level 180.
Begin Hero & Daughter and speak to Dr. Montague on Pioneer 2 before going down to Ragol. He will ask you to gather some data on Dark Falz.
Clear every area and defeat Dark Falz to gather the data Dr. Montague wants.
Return to Pioneer 2 and talk to both Principal Tyrell and Irene.
Talk to Dr. Montague on Pioneer 2 again. He will ask you to go back to Dark Falz.
Return to the Dark Falz arena to acquire the Old Ring.
Bring the Old Ring to Dr. Montague, and he will use his skills to transform it into a Red Ring.
The Twisted Towers

Orochi-Agito
You can convert a 1975 Agito into Orochi-Agito in this quest. The details are:
Complete the offline quest Seek my Master.
Begin The Twisted Towers and speak to the Tekker on Pioneer 2 near the hospital.
Agree to let him restore the 1975 Agito for you.
You will receive Orochi-Agito with the same percentages as the original 1975 Agito.
As with Seek my Master, you can perform one conversion per difficulty setting, so if you've already converted a 1975 Agito, you'll need to use a different difficulty setting or character to convert another one.

Quest Completion Prizes
You will receive a rare weapon for completing this quest in Ultimate mode. The weapons you can get are:
Hunter: Diska of Braveman, 30 Hit
Ranger: M&A60 Vise, 40 Hit

You will receive the prize once per character.
Parasitic Gene "Flow"
To acquire the Parasitic Gene "Flow", you must be playing in Ultimate mode. Follow these steps:
Follow the story path in the offline quests where you tell Sue your name in Dr. Osto's Research. This allows you to get the true endings in Soul of Steel and Seat of the Heart.
Complete Soul of Steel with the true ending and epilogue to gain Dr. Montague's trust.
Reach level 160.
Begin The Ends of Desire and speak to Dr. Montague on Pioneer 2 before entering the VR simulator. He will ask you to speak with him again after you gain access to Ragol.
Complete VR Temple and VR Spaceship.
Return to the Lab and talk to both Natasha and Elly to unlock the Ragol teleporter.
Talk to Dr. Montague on Pioneer 2 again. He will ask you to collect a DNA sample from one of Dr. Osto's experiments.
Complete Central Control Area and Seabed, then defeat Olga Flow to gather the DNA sample Dr. Montague wants.
Return to the Lab and talk to both Natasha and Dan.
Bring the DNA sample to Dr. Montague. He will give you Parasitic Gene "Flow" as your share of the specimen you collected.
